Susong Dermatology is Chattanooga’s premier dermatology specialists. Since Dr. C. Rodney Susong founded the practice in 1985, we have been privileged to care for hundreds of thousands of patients in the Chattanooga area. We offer medical dermatology, pediatric dermatology, dermatologic oncology including surgical dermatology.

In order to make your first visit as pleasurable and efficient as possible, please complete our new patient forms and questionnaires prior to your scheduled visit. You may complete these forms by:
**Please arrive 30 minutes prior to your initial visit so that you may be properly registered and acquainted with our facility and staff.
All patients under the age of 18 years of age must be accompanied by a parent or guardian at the first visit. If the minor is coming with someone other than a parent or legal guardian, a Minor Consent form must be completed and signed by the parent or legal guardian in order to seen.

Please allow 7 business days for us to release your records.
At Susong Dermatology, we make every effort to provide you with the best care possible and the most convenient financial options. To accomplish this, we accept medicare, most major insurance plans and credit card payments. Please contact our office to see if we accept your insurance plan.
Because your health insurance is a contract between you and your insurance company, benefits received are based on the terms and limitations of your insurance plan. We will be happy to file a claim form with eligible insurance carriers in order to achieve reimbursement for you, but please remember that you are responsible for all co-pays, co-insurance, deductibles, and non-covered services. We encourage you to read your insurance company policy and know the coverage or limitations of your medical insurance plan. Some Managed Care benefit plans require a specialist referral or prior authorization. It is your responsibility to see that a referral is obtained prior to every visit. If we do not have a referral, you will be responsible for full payment on the day of service.
For your convenience, we accept Visa, MasterCard, Discover, and American Express. Subject to credit approval, we also offer CareCredit. Payment is due at the time service is rendered unless other arrangements have been made in advance.
Our goal is to provide quality, individualized medical care in a timely manner. No-shows, late shows, and
cancellations inconvenience those individuals who need access to medical care. We would like to share with
you our policy regarding missed appointments or cancellations.
As a courtesy and to help patients remember their scheduled appointments, Susong Dermatology sends text message, voice message and email reminders 5 days, 2 days, and on the day of your scheduled appointment.
By selecting a response on our automated appointment reminder system (1 to confirm, 2 to reschedule, 3 to
cancel), this lets us know if you plan on keeping your scheduled appointment time.
In order to be respectful of the medical needs of other patients, please be courteous and contact our office
promptly if you are unable to show up for a scheduled appointment. If your schedule changes and you cannot
keep your appointment, please contact us at least 24 hours in advance so we may reschedule you and
accommodate those patients who are waiting for an appointment. If you need to cancel and/or reschedule
your appointment, you can do so by contacting our office and speaking directly with one of our scheduling
staff, leaving a detailed message with our scheduling staff, or selecting a reschedule or cancellation response
on our automated appointment reminder system.
A cancellation is considered to be late when the appointment is cancelled without a 24 hour advanced notice.

A “no-show” is a patient who misses an appointment without cancelling it. A failure to be present at the time of an appointment will result in a charge to the patient’s account.
Missed appointments may be charged $50 for an office visit and $100 for a surgical visit.
This “no-show fee” is not reimbursable by your insurance company. You will be billed directly for it. No further appointments will be scheduled until this no-show fee has been reconciled.
Susong Dermatology reserves the right to terminate the patient-doctor relationship if a patient has three (3) or more missed appointments
